Approaches Used

  • Medication Management
  • Psychoeducation
  • Supported Living Skills

What this often looks like

  • Hearing or seeing things that others do not, whether or not you believe they are real
  • Convictions that others around you find impossible, and that feel self-evident to you
  • Thoughts that will not line up into sentences the way they used to
  • Withdrawal from people, work and interests over months rather than days
  • A sense that something in the world has shifted meaning without explanation

What the first appointment covers

The purpose of the first appointment is not to argue with your experience. It is to understand what has been happening, over what period, what helps, what makes it worse, and what previous treatment did or did not achieve. Sleep, substance use and physical health are covered too, because each of them changes the picture substantially.

How it is treated here

Antipsychotic medication is the core of treatment, and the practical question is rarely whether to use one but which, at what dose, and with what side effects you are willing to live with. That trade-off belongs to you and is revisited openly. Long-acting injectable options exist and suit some people better than daily tablets. Physical health monitoring is part of the plan, not an afterthought.

What to expect, and when

Some effect on agitation and sleep appears within days to weeks; the fuller response to hallucinations and delusions typically takes several weeks. Negative symptoms — withdrawal, flatness, loss of drive — improve more slowly and are the part most often left untreated because they are quieter.

Questions patients ask

Can this be managed by video?

Stable outpatient care, yes: evaluation, prescribing, monitoring and regular review. Acute psychosis with risk to safety needs in-person and sometimes hospital care, and that is said clearly rather than managed at a distance.

I stopped my medication because of the side effects. Can that be handled differently?

Usually yes. Stopping because of side effects is a treatment problem, not non-compliance. Different agents have very different profiles for weight, sedation and movement effects, and switching is a legitimate, planned step rather than a last resort.

Will I be able to work?

Many people do, particularly when treatment starts early and continues. That is the honest answer: it depends on the individual course, and it is a goal we plan for rather than a promise I can make.
